2018 Community Health Needs Assessment Survey

thoughts regarding health needs for you, your family and neighborhood.

The survey will take approximately 10 minutes and all responses are kept completely anonymous. Your feedback will help inform future health care programming and services for your community.

Robert Wood Johnson University Hospital (RWJUH) Somerset in collaboration with Healthier Somerset is presently working on the 2019 – 2021 Community Health Needs Assessment.  The assessment entails the collection of secondary data, key informant interviews, focus groups and a community needs survey.   The community survey plays an important role in capturing the current health needs from Somerset County residents.  As part of Healthier Somerset, we need your assistance in distributing this survey to the community and encouraging them to take a few minutes to complete it.  Community engagement is key toward health promotion and improvement.
